#320 A3: widen linkage table to funcall + setf/fdefinition call classes (Axis A / ADR 0032)

Part of #318 (epic). Axis A, continues ADR 0032. Depends on A2.

Goal

Widen the module-local linkage table (from A2) to the remaining warm call classes still early-bound: funcall dispatch and the setf/fdefinition boundary.

Notes

• Preserve ADR 0032 notinline/setf/fdefinition semantics exactly.
• COLD path unchanged (early-bound; self-host fixed point must not depend on a mutable slot).

Acceptance gate

Same flat fixed-name/growing-site A/B as A2, extended to funcall-heavy and setf-heavy modules. Suite-green incl. asdf load.
